Certified Agricultural Producer Application to Sell

Thank you for your interest in selling at the East Palo Alto Community Farmers' Market. Please complete the following online application. (For a paper application, please click here.) Note that your application is not considered complete until you submit the following items:

  • Copy of a current Certified Producer Certificate
  • Certificate of liability naming Collective Roots as an insured party

About Your Farm

If yes, be sure to submit a copy of your organic certification.

If yes, be sure to submit a copy of your sellers’ permit.

No WIC ID number? Please let us know in the comments section at the end of this form, so that we can help you get one.

Selling for Another Certified Producer

If yes, be sure to submit a copy of the Certified Producer’s Certificate and a letter from the Certificate holder authorizing you to sell on his/her behalf.
